چکیده

Sodium hyaluronate was first used in ophthalmic surgery as a vitreous substitute more than 10 years ago' when its properties were shown to be elastic, highly viscous, yet nontoxic, noninflammatory, and nonantigenic.2 More recently Balazs, Miller, and Stegmann introduced its use into anterior segment surgery.35 The 1% solution (Pharmacia UK Ltd, Healonid) is at least 400000 times more viscous than aqueous yet will pass through the fine Rycroft cannula. It is totally transparent, and because of its high molecular weight it is nonosmotic. Because of these properties and the fact that it will maintain anterior chamber depth when air may not, it has been described as the ideal medium for closed chramber surgery.6 The particular assets of Healonid are that it will allow complex surgical manipulation in the anterior chamber, improving visibility while protecting the corneal endothelium and surrounding tissues from the trauma of surgical instrumentation. It may also be used to aid atraumatically the cleavage of iris synechiae or other adherent tissues. Thus Healonid has a place in the field of trauma'8 (synechiolysis, extraction ofcorneal splinters penetrating the anterior chamber, anterior segment reconstruction), in keratoplasty459 (both for donor and recipient eye to protect endothelium), in filtration surgery' (to prevent flat anterior chamber and hypotonia), in extracapsular cataract extraction'6 (during capsulectomy and to protect endothelium from a hard lens nucleus),

Role of sodium hyaluronate (Healonid) in triangular flap trabeculectomy.

The role of sodium hyaluronate (Healonid) in trabeculectomy to prevent a shallow or flat anterior chamber and hypotonia in the immediate postoperative period is reported. Twenty-nine eyes of 27 patients were included in a randomised controlled study. Thirteen eyes had trabeculectomy alone, and 16 eyes had trabeculectomy performed with Healonid injected into the anterior chamber. The results sho...

Pupil size and reactivity following hydroxypropyl methylcellulose and sodium hyaluronate.

AIMS A prospective randomised clinical trial was set up to compare the effect of hydroxypropyl methylcellulose (Ocucoat) and sodium hyaluronate (Healonid) on pupil size and reactivity following their use in cataract surgery. METHODS Pupil measurements were recorded before and 6 weeks after surgery. RESULTS There was no significant difference between the two groups with respect to pupil size...

Stabilization of flat anterior chamber after trabeculectomy with Healon5.

A 64-year-old phakic patient had a trabeculectomy complicated by complete shallowing of the anterior chamber 10 days postoperatively. Despite repair of a conjunctival wound dehiscence and reformation of the anterior chamber with sodium hyaluronate 3.0%-chondroitin sulfate 4.0% (Viscoat), he had a flat anterior chamber again the following day with 360 degrees of pupillary synechias and lens-to-c...

Viscodelamination at the vitreoretinal juncture in severe diabetic eye disease.

Injection of 1% methylcellulose or 1% sodium hyaluronate (Healonid) was used to separate attached vitreous cortex and fibrovascular epiretinal membranes from the retina in 40 eyes undergoing closed microsurgery for severe diabetic eye disease. The viscodelamination technique was of great value in elevating vitreous cortex or sparsely vascularised epiretinal membranes, especially in eyes with co...

SCIENTIFIC REPORT The Finger iridectomy technique: small incision biopsy of anterior segment tumours

Aims: To develop a minimally invasive, maximally effective method to biopsy anterior segment tumours. Methods: A 25 gauge aspiration cutter (vitrector) was used to biopsy anterior segment tumours. The probe was introduced under sodium hyaluronate 1% and through a 1 mm incision. Aspiration (600 mmHg) cutting (300 cpm) was performed to obtain specimens for cytology and histopathology. Results: Di...
